Nobody thinks about safety paperwork until an OSHA compliance officer is standing in the trailer, or a claims adjuster is asking for the orientation roster for a guy who twisted his knee eighteen months ago. Why the paperwork fails, and it's almost always the same three ways
Before we talk about what to keep, it's worth naming how safety documentation actually breaks down in the field, because if you don't fix the failure mode, a fancier system just fails the same way faster.
- It's collected once and never refreshed. The sub's certified welders and their fall-protection competent person were verified at mobilization. Then the crew turned over twice, and now half the guys on the deck were never oriented and their certs expired in the meantime.

The pre-mobilization stack I'd want in hand for every trade partner:
- Their written site-specific safety plan, or their corporate program mapped to your project. Read it. A generic program pulled off the internet with another GC's name still in the footer tells you exactly how seriously they take this.
- Their EMR (experience modification rate) and the last three years of OSHA 300A logs. An EMR under 1.0 means they've historically had fewer losses than the industry average for their class; north of 1.0 and climbing is a signal to ask questions and watch them closely, not necessarily to disqualify.
- Certificates of insurance with the right additional-insured endorsements and limits, and confirmation their workers' comp actually covers the workers who'll be on your deck, not just the office.
- The list of certifications the scope requires, with copies: operators, riggers/signalpersons, competent persons for scaffold, excavation, and fall protection, plus whatever the trade demands (confined space, hot work, etc.).
The competent-person piece trips people up. OSHA doesn't just want a competent person, it wants a named, qualified individual with the authority to stop work, for each activity that requires one. "The foreman handles it" is not a documented competent person. Get the name and the training that backs it.
Orientation: the record you'll be asked for most
Site orientation is the single document most likely to be pulled in a claim or an audit, because it's the proof that a specific worker was told about the specific hazards of your site on a specific date. Make it worker-level, not company-level. "ABC Electric was oriented" is worthless if the injured apprentice started three weeks after that session.
A defensible orientation record ties one worker to one date with their signature or a digital acknowledgment, and captures what was actually covered: your site rules, emergency and muster procedures, the hazards unique to this job, and the trade-specific risks for their scope. Keep the roster tied to the person, so when a name comes up you can produce their orientation in under a minute instead of digging. The daily and weekly cadence: JHAs and toolbox talks
Job hazard analyses and toolbox talks are where the paperwork meets the actual work, and where they should be genuinely tied to what's happening that week, not filed as a formality.

Records retention: keep them longer than you think you need to
Don't purge safety records when the job closes out. Some have legally mandated retention periods, and OSHA requires certain medical and exposure records be kept for the duration of employment plus thirty years, and claims can surface long after the ribbon is cut. Injuries from noise, silica, or other long-latency exposures can generate a claim years down the road, and the record you kept, or didn't, is the whole case.
