Ask most superintendents where safety lives on their project and they'll point at the trailer wall: the JHA binder, the toolbox-talk sign-in sheets, the OSHA 300 log. All of that matters. But the incidents I've watched happen over the years almost never came from a missing form. They came from two crews ending up in the same square of concrete at the same time because nobody planned it that way. A drywaller stocking board under a mechanical crew that was about to open up a lift. A concrete pour that ran late and pushed the finishers into the dark. Safety failures are usually coordination failures wearing a hard hat.

The problem is that the field doesn't run one activity at a time. It runs twenty, stacked in the same building, and the danger lives in the overlaps that no single trade's plan ever considers.
Think about the ones that actually bite:
- Stacked trades in the vertical. Anyone working overhead — steel, deck, MEP rough-in on a lift — over anyone working below. Nobody drew that on purpose. It fell out of two trades independently promising the same area for the same week.
- Schedule pressure translating into shortcuts. When a crew is behind, the fall-protection tie-off gets skipped, the ladder gets used one rung too high, the guardrail comes out early so material can move. The root cause isn't the worker; it's a plan that never had a realistic buffer in it.
- Work starting before the area is genuinely ready. A crew shows up to a floor that still has open floor penetrations, no lighting, or a slab that isn't cured, and they improvise. Improvising is where people get hurt.
- Silent adjacency. One trade cutting with a saw and no dust control right next to a crew that was never told, or a painter spraying solvent-based coating in a space someone else is about to weld in.
None of these show up on an individual pre-task plan. All of them show up the second you lay next week's activities out by location and look at what's sharing space and time. That's the whole game.
Put a hazard pass into the weekly work plan
The weekly work planning meeting is where reliable commitments get made — each foreman says what they'll get done next week and what they need to do it. Add one deliberate pass to that meeting: a hazard read of the plan you just built. It takes ten minutes and it's the highest-leverage safety time you'll spend all week.
Walk the plan location by location, not trade by trade, and ask three questions at each area:
- What new hazard does this week's work introduce here? Open leading edge, energized equipment, a crane pick, a confined space, a chemical.
- Who else is committed to this same area or the space directly above or below it? This is the one that catches the stack-up. If two crews are in the same location, either separate them in time or plan the protection between them — overhead barriers, exclusion zones, a hard sequence — before either one starts.
- What has to be in place before this can start safely, and who owns it? Fall protection, barricades, a permit, ventilation, an inspection. Every one of those is a constraint, and it gets treated exactly like a material or information constraint.

An activity that's truly "ready" is one that can be done safely. So put the safety constraints on the same list as everything else instead of letting them ride along informally:
- Fall protection designed and materials on hand for any leading-edge or elevated work — not "we'll figure it out," but anchors located and a plan drawn.
- Safe, lit, and cleared access to the work area, including how material gets there without crossing another crew's live zone.
- Permits pulled and posted: hot work, confined space entry, energized work, excavation.
- The right people trained and current for the specific hazard — you don't want to discover on the morning of the pick that your signal person's certification lapsed.
A constraint that isn't cleared means the activity doesn't get committed for the week. That's the rule that gives make-ready its teeth, and it works identically for a missing scaffold inspection and a missing set of shop drawings. Track scaffold tags, equipment inspections, and permit expirations right alongside submittals and RFIs, because to the schedule they are the same kind of blocker.
Sequence the hazard out when you can
The cleanest way to protect a crew from an adjacent hazard is to make sure the hazard isn't adjacent. A lot of multi-trade danger can be designed out at the planning table just by not letting two conflicting activities share a window.
A few that come up constantly:
- Overhead versus below. If you can't separate them in space, separate them in time — the crew below finishes and clears before the crew above opens up, or a hard overhead-protection deck goes in first. Don't let "we'll just tape it off" be the plan for a full week of exposure.
- Hot work near flammables or fresh coatings. Sequence welding and cutting away from the painters, or vice versa, and make sure the fire watch and clearances are real, not assumed.
- High-dust or high-noise operations. Sequence saw-cutting, grinding, and chipping so the crews that would be breathing or standing in it aren't scheduled into the same room, and give a heads-up to anyone nearby.
- Deliveries and material movement. Big picks and lift traffic coordinated so you're not running a loaded forklift through a corridor where a crew is on their knees setting anchors.
When separation genuinely isn't possible — and sometimes it isn't — then the plan owns the protection: the barrier, the exclusion zone, the spotter, the sequence. The difference between a planned adjacency and an accidental one is that somebody thought about it and someone owns the safeguard.
Carry it into the daily huddle
The weekly plan sets the frame; the day is where reality shows up. Spend the first two minutes of the daily huddle on the same hazard read, scaled down: what's the specific exposure today, what changed from the plan, and did anything new land in a shared area. Schedule changes are where fresh hazards sneak in — a crew that slid a day now overlaps one that didn't, and the conflict you cleared on Monday is back on Thursday.

And when weather moves — wind on a pick, ice on steel, heat on the crew — the huddle is where you re-plan the day around it rather than pushing through because the schedule said so.
Treat a safety commitment like any other promise

"We'll be safe out there" is not a commitment. "The guardrail on the east leading edge is up and inspected before my crew starts Tuesday, and I own that" is. Learn from what went wrong through the planning lens
When something does happen — an incident or a good near-miss — run it back through the plan, not just the JHA. The questions that actually change behavior are scheduling questions: Was this area truly ready, or did we commit it while a constraint was still open? Did two crews end up sharing space because we didn't catch it in the weekly read? Was the crew rushing because the plan was never achievable?
